your return to functional and recreational pursuits. At the time of your first visit, your physical therapist will take a detailed history and perform an initial evaluation based on your physician’s diagnosis. During the course of your treatment, the same physical therapist will continue to assess your progress and monitor your program. Your physician will be notified of your progress with consistent personalized reports. When your doctor refers you for physical therapy, you have the right to choose the facility that is best for you. In the state of New York, a licensed physical therapist may provide a patient with treatment in the practice of physical therapy without a prescription from your medical practitioner for ten visits or thirty days, whichever occurs first. Your health insurer may or may not cover physical therapy services without a prescription. It is advised that you contact your health insurer to determine if a physician prescription is required to reimburse for physical therapy services. A physical therapist who provides treatment, without a referral, will advise the patient, in writing, prior to treatment, of the possibility that such treatment may not be covered by their health insurance plan. The treatment may be a covered expense if a referral is provided. Our office participates with many insurance plans..
